Back to School Community Partner Registration Form

Thank you for supporting the Martin Luther King, Jr. Community Center! 

Community Partners can register and participate at no cost. You will receive one (1) table and two (2) chairs. You are expected to provide 1000 pre-packaged items to participate. 

You can also choose to donate items to the MLK, Jr. Community Center, which will be stuffed into backpacks and distributed to each student. The deadline to donate items is July 31, 2026.

You can also choose to make a financial contribution towards the purchase of items. Please contact our office directly for more information.

Release and Waiver of Liability

By submitting this form, you certify and agree to the following:

  1. You are at least 18 years of age and agree to abide by all instructions given by the managers or directors of the Event, or their agents. You understand and assume the risks of participating in this Event.
  2. You expressly release the City of Dallas, its officers, employees, and agents, from any liability for injury or death and/or damage to your property resulting from participation in the event, regardless of whether the damage or injury was caused and/or contributed to in any way by the negligence or contributed to in any way by the negligence or fault of the City of Dallas, its officers, agents or employers.
  3. You understand and agree that execution of this Release and Waiver of Liability does not constitute a release or waiver by the City of Dallas of the defense of governmental immunity, or any other legal defense, which may be applicable with respect to any claim, which may arise against the City of Dallas.
